Nowhere without a lawyer. It’s just not going to happen that a POA will be revoked based on what you think.
Try Legal aid

You will need to sue for custody or a change of medical power of attorney. If the abuse becomes criminal, it will be reported by the nursing home.

You might try speaking to someone in Legal Aid or someone with Adult Protective Services.

You need to go to court and file to have a new guardian or conservator appointed for your grandmother. As part of that proceeding, you would ask the court to rescind the POA your mother has. You may not be able to afford an attorney, but you need one.
Like wizjip said, try legal aid. You can also call the state bar association where you live (look them up online or in the phone book) and ask if they have any pro bono services (or reduced fee services) for persons who need assistance.
